This is a Fred Perry track jacket, model J6231.
It's made of 46% cotton and 54% polyester, and manufactured in Portugal.
The body is blue, with white stripes on the sleeves featuring the laurel logo.
The size is S, but it's a larger overseas size, so it will fit someone who wears a size M.
